RESUMO

Rationale: Ameloblastoma often requires complex facial treatments such as resection and reconstruction, which can be aided using three-dimensional resources towards improved surgical planning, reduction of surgical time, and customization. Patient Concerns: A 51-year-old patient, concerned about submandibular volumetric increase discovered performing imaging exams to install dental implants. Diagnosis and Treatment: Diagnosed with ameloblastoma, he underwent uneventful partial mandibular resection and placement of prebent mandibular reconstruction plate. Four months later, following a motorcycle accident, a deflection of the plate and mandibular fracture was observed. Using the initial prototyping, a new reconstruction plate was customized and surgically placed to reduce the mandibular fracture, with adequate titanium plate adaptation to the remaining bone and maintenance of aesthetic mandibular contour. Outcomes and Take-away Lessons: The current digital dentistry resources, such as computed tomography and intraoral scan, can be recovered and used as a backup of recorded anatomy anytime in future, providing a long-lasting facial digital biobank.

PURPOSE: Virtual surgical planning (VSP) has become routine practice in orthognathic treatment planning; however, most surgeons do not perform the planning without technical assistance, nor do they routinely evaluate the accuracy of the postoperative outcomes. The purpose of the present study was to propose a reproducible method that would allow surgeons to have an improved understanding of VSP orthognathic planning and to compare the planned surgical movements with the results obtained. MATERIALS AND METHODS: A retrospective cohort of bimaxillary orthognathic surgery cases was used to evaluate the variability between the predicted and obtained movements using craniofacial landmarks and McNamara 3-dimensional cephalometric analysis from computed tomography scans. The demographic data (age, gender, and skeletal deformity type) were gathered from the medical records. The data analysis included the level of variability from the predicted to obtained surgical movements as assessed by the mean and standard deviation. For the overall sample, statistical analysis was performed using the 1-sample t test. The statistical analysis between the Class II and III patient groups used an unpaired t test. RESULTS: The study sample consisted of 50 patients who had undergone bimaxillary orthognathic surgery. The overall evaluation of the mean values revealed a discrepancy between the predicted and obtained values of less than 2.0 ± 2.0 mm for all maxillary landmarks, although some mandibular landmarks were greater than this value. An evaluation of the influence of gender and deformity type on the accuracy of surgical movements did not demonstrate statistical significance for most landmarks (P > .05). CONCLUSIONS: The method provides a reproducible tool for surgeons who use orthognathic VSP to perform routine evaluation of the postoperative outcomes, permitting the identification of specific variables that could assist in improving the accuracy of surgical planning and execution.

RESUMO

Obtaining a good cosmetic and proper mastication in oral rehabilitation with dental implants are directly linked to the correct positioning of the implant in the alveolar bone. The malposition of the implant is a challenge in rehabilitation, which can often compromise the entire process. In cases of severely malpositioned implants, one has the option to remove it or leave it submerged under bone and gums. Another option is the modified segmental osteotomy that aims repositioning osseointegrated implant with the surrounding bone. The objective of this case report is to present a case where an implant was severely malpositioned after surgically assisted rapid maxillary expansion, requiring a modified segmental osteotomy technique to reposition it.

RESUMO

Traumatic lacerations to the skin are problems frequently seen and treated by emergency centers around the world. Among all wounds, dog and cat bites are commonly seen. As in many mammals, different species of microorganisms are found in dog and cat mouths with a potential pathological effect to humans, as represented by rabies. The injuries have disfiguration effect with possible psychological repercussion to the patient. This article aimed presenting up to date considerations regarding the management of animal bite injuries to the face, exemplified by a case report that should be the interest of all professions that deal with facial tissues, as dentists do.
